Rostering a team

Registration Spring 2016

Below is information for Registering your team:

  • If players register after April 1, 2015 their USA Confirmation is good until August 2016.
  • If back ground checks were done in 2013 Coach’s must re-do background check
  • Safe Sport, is done every 2 years.  If renewal is required go online and re-do approx. (30 min) If the safe sport was done after April 1, 2015 you should be good for 2 years.
  • Birth Certificates please do not send with your information, if I need it, I will let you know.

The following is what will be needed to generate your roster for the Spring 2016 season:

  1. All players and coaches must be registered on APHA before I can create the Excel Spreadsheet for the roster.
  2. Up to date Safe Sport Certificate http://www.usahockey.com/safesporttraining
  3. Background Check (make sure that the receipt shows the $10.00 paid) the following will not be accepted passed, clear etc.  http://www.maha.org/page/show/1284902-background-check  
  4. To get a new copy of your background check:   Go to: http://maha.sportngin.com/user_dashboard/show and Log In with your email and password, once logged in click the dropdown next to your username in the upper left corner of the site, select Purchased Items, click on the Order link, next click the entry link, from the Entry click Print Entry in the upper right corner.
  5. If safe sport and background check is not submitted for the coaching staff, they will not be able to be put on the roster.  This will cause a roster delay!
  6. If you have a player that needs a transfer, the transfer has to be done before the player can be placed on the roster! NO TRANSFER, PLAYER WILL NOT BE PLACED ON THE ROSTER!

 

Once your roster has been generated, it will be sent for final certification.   Once I receive it back I will send you the link.  Open the link and print your roster.    NOTE:  Any time there are changes to your roster, the Link will be updated.

District Credentials (Paperwork needed) Squirts and older.

  • Roster - Stamped approved.
  • Background checks for any coaches on the roster
  • Birth Certificates for any players that don't show as verified on the roster
  • Each of the following in Roster order (Coaches separate)
    • Consent to Treat (including coaches)
    • Parents Stars Report
    • Participants Code of Conduct (including coaches)
    • Concussion forms
